The U.S. Department of Commerce, Bureau of Industry and Standards issued Export Administration Regulations, following Title 15 of the Code of Federal Regulations Parts 730 through 774.
Some items are on the Control List, please consult our website under product information. If you would like to purchase an item that is controlled we will obtain for you, an Export License that is valid for 4 years. Please complete an Export License Statement. If you are purchasing Botulinum Toxin or Staphylococcal Toxins a Letter of Assurance also needs to be signed and returned. Once we have received the required documents we submit an application to the U.S. Department of Commerce to review. It usually takes 4 weeks or longer for them to issue a license. Since the license is issued for a 4 year period, we apply to transfer multiple quantities of the items you intend to purchase for the duration of the license.
A $50.00 fee is added to each shipment of a controlled product (also detailed in fees).
List Labs ships most products via Federal express. Select Agent products can only be carried internationally via World Courier. For these, it can take 1 to 2 weeks for the shipping documentation to be approved by the carrier and the destination country’s customs agency. The purchaser is responsible for duties and taxes as well as these transportation fees.
When purchasing a controlled item that requires an Export Certificate by the U.S. Department of Commerce, a $50.00 fee is added to each shipment.
A $50.00 processing fee is added to international orders that do not require an export license.

Step 2. Submit a purchase order on company letterhead or an official purchase order. Once approved you will be notified and your order will be processed.
Please Note: Large purchase of Select Agents require permission from the Centers for Disease Control. In this situation, Form 2 is required. Entities that are registered with the Select Agent and Toxins program are familiar with this requirement. See the details below.
List Labs is registered for the Select Agent Program by the Centers for Disease Control. Botulinum neurotoxins and Enterotoxin type B from S. aureus transfers are controlled by the guidelines set forth by the CDC. A hard copy of a purchase order is required and a Letter of Assurance. Some quantities require A Form 2 approval by the CDC.
